1. Full time staff that answers the phone Monday-Friday 9:00am-5:00pm daily
2. One hour pre-certification of your patients insurance benefits. (Provided we can contact insurance).
3. Guarantee that the products we offer are in stock and ready to ship. (Except for Manufacturer Backorder).
4. Free sensor for your Medicare Patients with Medicare purchase.

To arrange a home rental unit:
Contact us at (800) 382-5879 and request one of the units from our DME rental page. Each unit you receive from
Current Medical Technologies will have a S.A.S.E. with the following paperwork:
A. Patient Information Sheet
B. Certificate of Medical Necessity
C. Therapy Agreement w/Patient Copy
D. Credit Card Authorization Slip. Must be included or unit may not be released.
(Physical Therapist is responsible for unit until paperwork is received).

Copies of this paperwork are easily downloadable below
1. Fill out all appropriate paperwork with patient signature and dispense unit to patient.
2. Clinician is responsible for mailing or faxing paperwork back to CMT, not the patient.
3. CMT will bill insurance or patient directly with no other clinician involvement.
